Mega Forms might be a bit more popular than Jotform. We know about 4 links to it since March 2021 and only 3 links to Jotform. We are tracking product recommendations and mentions on various public social media platforms and blogs. They can help you identify which product is more popular and what people think of it.
The fields parts can probably be done with any plugin, but for the custom emails and the emails conditional logic, I think you'll need to get a premuim copy of one of the popular form builders. If you would like to use a free option, I suggest Mega Forms, it supports all the fields you need ( checkbox, radio, textbox, pdf/file upload..etc ). For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). If you want to go with a plugin I suggest Mega Forms, it allows you to create forms, send notifications, create conditional rules and also has AJAX submission (Disclaimer: I'm the developer). Something akin to a fill-in-the-blank template maybe? This was all prompted by my beginning to write one on jotform.com.
