  Https://surveyjs.io/ - I can recommend this tool, I used it for perhaps few hundreds questions long survey with rules and different elements, and overall can say it's done the job well. First you compose form with visual editor on their site, then save JSON somewhere in project and the lib will use JSON to generate form with all controls, inputs, logic. Source: about 2 years ago
