Have you taken a look at freewill.com? You can do a revokable trust there as well, I think it would cut out a lot of the lawyer fees since it's free. You maybe have to get it notarized, I'm not 100% sure because I still haven't finished mine. Source: almost 4 years ago

If that is not an option, you could print out a form from a website like http://freewill.com and he signs it in front of a notary (usually you can find one at the bank). Then you definitely need Durable (valid when person is incapacitated; as well as when theyโre not) POA asap. Youโd be surprised how often we see โoh I donโt think heโs bad enough for us to get POA yetโ. Naturally, when an emergency happens and POA is actually needed, itโs already too late to acquire it. Maybe it would help to create a will yourself using freewill.com as recommended on this thread. All he would have to do is read and sign it. It would at least serve as a draft that he could react to.
