It is normally part of whatever system they are required to use in order to apply, yes. I don't know that there's a standard way to do it, as inclusion of that kind of language is usually overseen by legal and has a lot of weird caveats depending on where you're planning to collect data, where you're going to store it, etc. There are a bunch of services online that will generate text depending on your particular...
