    The fields parts can probably be done with any plugin, but for the custom emails and the emails conditional logic, I think you'll need to get a premuim copy of one of the popular form builders. If you would like to use a free option, I suggest Mega Forms, it supports all the fields you need ( checkbox, radio, textbox, pdf/file upload..etc ).     For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). If you want to go with a plugin I suggest Mega Forms, it allows you to create forms, send notifications, create conditional rules and also has AJAX submission (Disclaimer: I'm the developer).
