The fields parts can probably be done with any plugin, but for the custom emails and the emails conditional logic, I think you'll need to get a premuim copy of one of the popular form builders. For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). If you want to go with a plugin I suggest Mega Forms, it allows you to create forms, send notifications, create conditional rules and also has AJAX submission (Disclaimer: I'm the developer). I think some of this might come down to stack as well. I watched a t3.gg video[1] recently about Convex[2] and how the nature of it leads to the AI getting it right first time more often. I've been playing around with it the last few days and I think I agree with him.
