    For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). If you want to go with a plugin I suggest Mega Forms, it allows you to create forms, send notifications, create conditional rules and also has AJAX submission (Disclaimer: I'm the developer).
