I can highly recommend magoosh.com (including their blog). They have a prep program with quite a lot of math problems with full explanations. I worked on improving my scores from ~162-163, and it went up all the way to 169 (I got 170 on but practice tests, but... well). Source: almost 5 years ago
As for GRE, I will preface my answer by stating that different people will have different strategies that work best for them. I started with all the common GRE practice books. When I say I had them all and went through all of them, I mean it. Unfortunately, those didn't work very well for me. What I found to be the most useful was magoosh.com.
