  I know that https://elevatedaccess.org/ has helped a few trans youth and their families move from Florida to NY (they partner with another org that assists with housing).   Hey OP, https://elevatedaccess.org/ is a group of private pilots that will fly you for free to where you need to get meds, doctor visits, etc. I would go ahead and get signed up with them.
