I'm not sure it covers all your features listed, but I use PhotoStructure [1] for the 'album' side of things. It's been mentioned a bit on HN, which is where I found it. I had a similar issue with photos/videos and ended up building a cli app to organize everything, it has worked for my use case relatively well, still, there are uncovered corner cases.
