As developers, we were frustrated by some of the shortcomings of the most popular feedback apps, so we simply decided to make our own with all the features and simplicity we were wishing for.

We used Feedbucket when launching our website. It made it easy to report bugs and improvement ideas directly on the page, and helped us work through them quickly.
