The fields parts can probably be done with any plugin, but for the custom emails and the emails conditional logic, I think you'll need to get a premuim copy of one of the popular form builders. For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). You mention YAGNI, but I think the bigger blocker is "Don't reinvent the wheel" - which I think is the most misunderstood bit of programming advice out there.
