The fields parts can probably be done with any plugin, but for the custom emails and the emails conditional logic, I think you'll need to get a premuim copy of one of the popular form builders. For a form with notifications, you should either develop this with PHP and HTML or use an existing plugin ( you'll need to build the logic, markup, security ..etc ). If you want to go with a plugin I suggest Mega Forms, it allows you to create forms, send notifications, create conditional rules and also has AJAX submission (Disclaimer: I'm the developer). You mention YAGNI, but I think the bigger blocker is "Don't reinvent the wheel" - which I think is the most misunderstood bit of programming advice out there. In my SvelteKit app, I am not using a "UI Framework" that provides components to me. I don't need to. Svelte is simple enough that I can build almost anything myself.
