Time Out is free, but you can donate for extra options, which I personally do. This is one of these succesful macOS apps that just keeps being updated every year or so. Pomodoro works ok for me, but definitely feel free to customize it! I do 45 minutes of work and a 20-30 minute break. The break may seem excessive, but it's what works best for me after lots of trial and error. I feel like I'm productive enough to justify it (anyway, without it I'd be getting nothing done lol).
