I am actually Italian and also there healthcare is public, in Germany it's a mix. When out of Europe, one has just to buy an international insurance, most of them cover worldwide, others cover macroregions. There's a bunch of replacements. I like https://newsblur.com but there are 4-6 large-ish similar sites. That said, partially what people miss is the relative cultural hegemony of Google Reader.
