For the character sheet, I took screenshots of the sheet from our current campaign and just slapped them all into one big image. We're playing in the Foundry Virtual Tabletop software with the Tidy5e and Inventory+ modules. I think those are the main ones affecting the appearance and layout of the character sheet. Looks like you can still buy if from the company that make it (Lone Wolf Developments, wolflair.com) but it doesn't look like it's been updated in a while.
