    Most todo management systems want to be able to cater to GTD, but they don't want to limit themselves to it because they want to attract non-GTD customers. The only app I've found that I think sticks really close to GTD is FacileThings.     If you are OK with a paid service hosted online -- and it sounds like you are, since you experimented with Todoist and Nirvana -- then I think Facile Things might tick all your boxes. I used it for years and was very happy with it. I dropped it only because for various reasons it became important to me to be able to keep everything locally and work with it offline.
