  Yes, if they keep your DMs open and you change your name it will update for them, and all of your previous conversations it will update your name.   The easiest way to save a discord profile picture is to enable developer mode in advanced settings, go to someone's profile, click/tap on the three dots and choose "copy id".   Go to Settings -> Privacy & Safety -> Request data, then once it arrives and you've downloaded it go to the folder called "messages" and look for a folder named "c848640894764974090" (it's the dm channel id from your screenshot), inside the folder open the file called "channel.json" in some text editor and look at the field "recipients" with two ids: one will be yours, the other one will be your friend's.
