If you are looking for free setup - you can combine ACF plugin with Custom Post Types to create your products/services (or extend woocommerce products) and for forms use a Forminator (in free version it provides date-range field needed to select dates). Gravity Forms is a very reliable premium form plugin. If you can buy, you can go with it directly. If you want a free form plugin you can go with WPForms. But its Pro version is quite costly, so you might not want it if you will eventually buy the pro version.
