  3.The only way I've come across to document the amount of time spent is to use timesheets or time tracking softwares. Some examples of time tracking softwares are Toggl, Hubstaff, and Time Doctor.   There are plenty of desktop/mobile apps for working with markdown. (I've been using Notable (desktop) and GitJournal (mobile ) for an Evernote-like experience.)
