I am actually Italian and also there healthcare is public, in Germany it's a mix. When out of Europe, one has just to buy an international insurance, most of them cover worldwide, others cover macroregions. There are lots of places where all you have to do is upload your music, and the website will distribute/release it across a ton of platforms (Youtube, Spotify, Amazon Music, Apple Music etc.) I've used a place called DistroKid to do it, but there are a few others out there that do the same kind of deal.
