Aimed at those looking to manage and improve their mental health, Hemisphere provides a new way for users to record and track symptoms and manage their general wellbeing. Hemisphere utilises customisable questions and fields to help users log and track any element of their daily life, such as sleep, diet, exercise or social interaction. Unlike other mood and lifestyle trackers, users can fully customise an unlimited number of questions, select their response types and collect the information most suited to their needs.
We’ve designed Hemisphere so that users can record and gather information that is relevant to them, as we know that everyone’s circumstances, symptoms and lives are unique. We believe that this level of flexibility means users can tailor Hemisphere exactly to their needs.
The answers collected can be analysed and displayed so that users can look for any trends or factors that might be impacting their wellbeing, such as changes in sleep, exercise, or major life events. We understand that security around personal data and health is of the upmost importance. Hemisphere uses military grade encryption to encrypt all answers before storing them.
We also recognise that users support networks are vital to recovery and their general health, so we’ve built in custom alerts that allow them to alert family or friends if they fail to check in.

/u/slugshead mentioned Xibo. They've discontinued Pi-compatible versions but run on a lot of other things. Player(client) licenses per major version are perpetual, or at least very long. I've seen some weird issues with them, don't know if it's fixed (mostly has to do with display scaling, probably related to the consumer-grade smart TVs I have to run it on). There are several roll-your-own. For example, Xibo. To get it totally free, you'll need to fire up a Linux master server, then learn the UI for creating content. Source: about 2 years ago
