  I had assume it was Google itself because I hadn't heard of surveys being part of the AdWords program, but a quick google search showed I was wrong about that. However, I stand by my assessment that the inconsistency between the answers indicate it was an "AI Generated" question. (One option is a named DLC, another is a generic " DLC" answer, and the other two are just the base games' names). Source: over 2 years ago

The Gatsby docs do a pretty good job in explaining how to build a contact form and it provides options for form submissions.
