We're commonly doing this with logging, using services such as Loggly or DataDog. We're using managed databases, be it on AWS, Heroku or database-vendor-specific solutions. Koblime uses Sentry (https://sentry.io) to detect crashes and performance issues and PagerDuty (https://pagerduty.com) to send me an alert. The data tells me if an issue is isolated to a single region or user or if it's a site-wide outage.
