Truly distributed, fully decentralised Peer-to-Peer services (where the user is also a node/server) mean that you basically host your profile on your phone or computer, and it can communicate with other people's phones/computers. This is basically a level up from Federated - you're still federating but at an individual user level. Somewhat prototypical examples include Scuttlebutt and Briar.

Federated systems like Mastodon or Diaspora allow you to join a server of your choosing (or run your own). Although your profile exists on a particular server, you can interact with users on other servers, and pick your profile up and move it if you want. This is basically the same as e-mail. We don't think twice about whether alice\@example.com can send a message to bob\@mailprovider.com - it's all just email. This is in contrast to fully centralised platforms where everyone is @facebook or @twitter and you can't see twitter posts from facebook or vice versa.
