I use Wekan, an open source Trello clone. I create a board for each milestone with three lists of cards, To Do, Doing and Done. I've used this method for a couple multi-year projects and don't see myself needing to change it.

HacknPlan

I've also messed around with HacknPlan before, I'd recommend trying that, the free plan was perfectly fine for me. I stopped using it as I found I was only using the Kanban boards and didn't see the need for the extra game design tools. Also my slow internet connection encouraged me to shift to locally hosted services.
