  3.The only way I've come across to document the amount of time spent is to use timesheets or time tracking softwares. Some examples of time tracking softwares are Toggl, Hubstaff, and Time Doctor.   I was using LastPass for years and years but recently switched to Myki ( https://myki.com ) when LP suddenly changed (i.e. crippled) their free version.
