If you only need to add several fillable fields and that's it -- I'd go either with signnow.com or with pdffiller.com (they look different but this is the same, US-based vendor). Source: over 2 years ago

Notion doesn't have any structure out of the box. Think of it as an empty database with no tables or schema. It's up to you to either import templates or design a system that works for your needs.
