Transactional emails are sent through SMTP and bulk email newsletters are set via Mailgun's API which you can configure in the Ghost dashboard (Mailgun is the only option for this). Mailgun is pay as you go and costs $0.80 per 1000 emails and there are some free emails included... Source: about 3 years ago
