iSpring QuizMaker is an application for creating quizzes and tests that allows you to choose from a variety of questions types, add multimedia to questions and answers, provide students with feedback upon their answer choice, allow multiple attempts, limit test time, assign custom points and penalties, and even more. I'm not sure it covers all your features listed, but I use PhotoStructure [1] for the 'album' side of things. It's been mentioned a bit on HN, which is where I found it.
