Godville KlinikiKliniki provided me with all the tools I need to manage my Physiotherapy clinic. Instead of having 10 subscriptions between accounting tools, statistic tools, patient management, appointment management, appointment reminders and others, Kliniki is an all in one solution that gives me all of that and more. What I loved most is their wallet system. Instead of subscriptions, I pay once a year to fill my wallet and totally forget about payments.

I believe https://godvillegame.com/ is very close to what you are aiming at. And they have pre-moderation that takes months (that is, your game has to have that much time). You can play as actively as you want, but you can also close the tab and not return for years, and your character will happily quest/level up/buy/sell/eat/fish/heal/revive/change guilds/get drunk/etc. On its own.
