You can also get it online, Either from apple.com or Pchome or momo. Both are authorized online resellers of apple. I bought my both phone, One 3 years ago (form apple) and one just yesterday (from Pchome) and few of my friends have done the same and had no problems whatsoever till date. You can still get it done without insurance, it's just really expensive. It will be thousands of dollars cheaper to get insurance for a year or so and have the surgery covered than to try and pay for it out of pocket, so it's worth maybe getting insurance.
