I've been looking for a good set up for this as well. While not exactly what you were looking for, it's worked well for me. For tasks, I use taskwarrior. It's not vim related, just a CLI tool, but it is so powerful even though it is really pretty simple. For notes, I do all my notetaking in vim and just use a markdown format and markdown preview for rendering if I want to view it while taking notes live. It's nice to use a simple standard format (markdown), be in complete control of the markdown files (can put into git repo and encrypt and store on some cloud storage if you want), and is really simple.
