Yes, I use this kind of a setup. The main tool in my setup is the Zotfile extension to Zotero. I attach PDFs as file links to their entries in Zotero, and have configured Zotfile to keep everything organized by type of entry. Zotfile (zotfile.com) only extracts highlights and comments, not handwriting. Handwritten notes would be stored as an image in the PDF file. It would be impossible to determine that this is a "handwritten-note-image" and not just a image that is part of the original PDF file. +1 for Zotfile for zotero http://zotfile.com/ the most important feature for me is that you can extract annotations from the pdf and save them as notes alongside zotero entry! I used it to highlight important snippets in the pdf (using Okular / FoxitReader ) and then search through them in zotero. Yes, it can be a tad clunky and the mobile app for a long time was just a catalog. However, their new iOS app [1] is awesome. For (2), I think no. You can use Zotfile to move attachments into folders. You can use Zotero Folder Import to import all attachments in folders into your Zotero library. I have a similar setup (Zotero + dropbox). Zotero syncs the entries and any notes, but dropbox takes care of the pdf storage. You can swap out dropbox for your storage of choice. So my workflow is similar to this. I read the paper and highlight the interesting parts. Then I use the zotfile plugin to extract the annotations and highlightes text from the pdf. I use Zotero for capturing articles (i.e. Saving the PDF file and adding the corresponding BibTeX entry). I use the Better BibTeX plugin along with Zotfile.
