italki is a language learning social network that connects students and language teachers.
Hire a tutor on iTalki. You can discuss your personal requirements with them before you book a lesson and can book in a taster lesson to see if you like them. On the taster lesson, I would ask them what plan they would put you on to ensure that you get the most bang for your buck i.e. Lots of scenario based conversation practice, doctor/patient etiquette in Japan, building up your vocabulary etc. I would also recommend you go through some kind of textbook/workbook so you can keep studying in your free time (The Genki series & the Japanese from Zero series are pretty popular).

You can also use a tool like Anki to make flashcards based upon the words and phrases you pick up that will be useful for you. If you're unsure what Anki is and what a Space Repetition System is have a look on YouTube for various tutorials.
